Sto svolgendo delle ricerche, per un progetto di tesi, Munchausen Syndrome by proxy and I need to know what are the psychodynamic implications of this disease. Furthermore, as is done when you are faced with this kind of problem? Where can I find more specific information and data on this? I look forward to your response as soon as possible! Sincerely

Answer Hello Claire, Munchausen syndrome by proxy is a form of abuse of a child abuser in which he does because he believes or try to pretend that that child has some form of disease that is suddenly cured. This syndrome involves, usually, the parents of the child over a long period of physical abuse. Let's see, first, what is meant by syndrome Munchausen. Baron Munchausen (who really existed) is the character of a history of GA Burger who fought alongside the Russians against the Turks. He then retired to a castle there and entertained his guests with stories and exaggerated stories and improbable. Hence the reference to the syndrome, by embodying the invention and exaggeration of facts or clinical symptoms reported by people who constantly turn to medical care. The expression was first used in 1951 by Asher and Meadow in 1977 used to preset the term "Munchausen syndrome by proxy" in reference to the fact that one parent, usually the mother, the child requires continuous assistance to physicians and hospitals, inventing or creating art to physical symptoms. In DSM-IV-TR (2001) this syndrome is listed as "Factitious Disorder by Proxy" and is noted as "The essential feature is the deliberate production or simulation of physical signs or symptoms in another person who is entrusted to of the subject. Typically the victim is a young child, and the person is the mother of the child. The motivation for this behavior is believed to be a psychological need to assume, through a third person, the sick role. " The characteristics of the abuser immediately catch the eye, because, generally, are people who have great familiarity with medicine (it was found that many of them are paramedics) solicit and interact in a continuous and consistent with hospital officials. Appear to be very caring and involved in the care of their child. Lastly, it appears that many have had similar experiences during their childhood. The other parent (usually the father), however, rarely led to abuse of the child. Often indifferent and passive, not cut out (or allowed them) a primary role in treating diseases of the child. Their hospital visits are infrequent and also is not able to stop the abuse of children by the mother. Using Bowlby's attachment theory, this type of mother-child relationship can lead to an attachment bond of type D, WARNING, disorganized-disoriented in which the child is faced with a paradox affective: the person who should care for is the same but was bullied and "choking" with excessive care and attention, and in case of his rebellion to these behaviors, and is blamed abused to a greater extent. The reaction of these children is that of a hyper-attentive and hyper-vigilance over their behavior and that of the mother, with the result of having a baby always frightened and that does not recognize, or refrain from bringing out their emotions, laying the foundation In this way, a style flat affect and little responsive to the attentions of others. At: Munchausen Syndrome find a psychological explanation of the syndrome Munchausen, in key system given by Dr. Paul Chellini Taking care of the child is viable through two routes: a. B. The protection Therapy.
the protection was achieved by involving the Prosecutor's Office, the Juvenile Court and the social services department, the instruments should be used and taken initiatives that protect the child from the recurrence of abuse and psychological pressures that can compromise detection (possibly calling for his removal from the family and his entry into the community with the necessary professional and human resources, or removal of the alleged abuser if the family is protective). also should be avoided too invasive diagnostic obstinacy (interviews ripetute,trattamenti coatti) volti ad ottenere rivelazioni più complete.

consigliaste sites where you can view the material on "Munchausen syndrome" in order to achieve a thesis on the subject. Sure of your availability, Best regards. Thanks

Answer Dear Joseph, first, about the Munchausen syndrome, may be curious to find out somehow Etymology: This symptom is named after the bizarre "Baron Maunchausen" really existed, a character in a major story GABurger who fought alongside the Russians against the Turks .. he retired to a castle there and entertained his guests with stories and exaggerated stories and improbable. From here, therefore, the reference to the syndrome, which is the invention and exaggeration of facts or clinical symptoms reported by people who constantly turn to medical care. Paolo Chellini, non tanto odio verso il figlio, quanto piuttosto pathological love .. do not need total control over the child rather than on the situation to him. In the meantime you can read all about to what, exactly, this response of Dr.. Chiellini, by Vertex Network, which examines the issue mainly through a systemic perspective:

The literature says that it is seldom a real women with mental illness: it can mean suffering from personality disorder Histrionic, Borderline, Passive-Aggressive, Narcissistic, Paranoid .. and in whatever way you want to label the issue it is still women who fight the feeling of emptiness inside assuming the role of devoted mother and mega ready to sacrifice the disease these women need to create a sort of good character and, for them, I needed to fill a fragile dall'autostima more than uncertain. Question Hello, I wanted to know what motivates a parent to slowly poison her child (Munchausen syndrome). I can not think of it as a gesture born of hatred against an unwanted child. This may be a desire at all costs, to exercise complete control of your life child? A sort of umbilical cord "infinity", where a mother, unconsciously wants a total power? Thank you.

La ricerca clinica su queste tipologie families shows that parents are people with low self-esteem, great difficulties in interpersonal relationships, a strong distrust of the news, etc.. From the above it is possible that these families together, to represent the parental figures (I think it is fair to extend the momentum even more generations: grandparents, aunts, etc..) Niches of collusion to keep peace at all costs. Try to imagine the "role" of the child in a similar dynamic: two subjects with a slightly different personality structure (ie with a low capacity for self-determination) from the respective families of origin create a new family. Penso, che a questo punto, ci siano delle buone probabilità che this type of behavior is something so rewarding that can be unconsciously sought by the mother. At this point, any push for autonomy will be experienced by the child's exploration of the mother as a loss of social status that can be recaptured only in times of illness. I suppose at this point, as a defense mechanism that pervasive anxiety that the mother will experience the loss of his state of caring person you will be able to generate some kind of idealized projection of reality. At this point the evolutionary pressures of the child will become signs of disease that only you can see and treat, in a kind of escalation where the relationship with her child will purchase for you and unfortunately, in some cases, for the whole family a kind of constraint and invischiante delusional. This is not to say that the dynamics I have described is a rule for the emergence of this syndrome, but unfortunately I think many of these pathological interactions that may occur within families, may be due to the factors I have described. I hope not to be too simplified description of this terrible syndrome that the record has brought us back to attention, but I think that this description may have some value relational reality. Let us always remember that complexity theory points out there with the "Principle of equifinality": that you can start from different assumptions and get risultati uguali, o partire da presupposti uguali e ottenere risultati diversi.

Secondo Conduct a search a decade ago, more than 5 percent of the contacts between doctor and patient would be carried out to Munchausen syndrome, which is something different dall'ipocondria shape as a deliberate attempt to deceive the medical staff and paramedico.La disease occurs almost always the patient from the need to attract attention to himself, to be the subject of care and concern by the GP and family and "exist" in the eyes of their relational world, as "a hero of the disease." When treatments that the patient is undergoing invasive or debilitating is possible to trace a masochistic component and autolesionistica. Sono state descritte somministrazioni di lassativi to produce diarrhea, pseudoematurie (often when the mother's menstrual cycle), pseudoinfezioni urinary tract by introduction of pus in the urine, or even pseudoglicosurie hypoglycemia by insulin injection, hypernatremia from abnormal salt intake, drug poisoning. The authors

after framed by a theoretical point of view of Munchausen Syndrome by Proxy Factitious Disorder by Proxy which according to the instructions provided by the DSM-IV, provide a discussion of the various forms in which this syndrome occurs, and the main most common means by which mothers confer the disease to their children.

especially for particular modes of manifestation and the involvement of children as
patients [1] [2] [3]. Exactly the same nature, sometimes incomprehensible,
of symptoms, this syndrome is associated with Munchausen syndrome, differing from it only for the diversity of actors involved, Munchausen Syndrome, in fact, has as its protagonist an adult pretending to be ill inventing a procession of symptoms of disease patterns evocative
true and proper conduct resulting in the creation of absolutely staggering.
This syndrome has its first definition by the scholar
Asher [4] in 1951 that the utilizza appunto in riferimento a quelle persone che si
disturbi frequenti e spesso inesistenti fino a riportare conseguenze estremamente
patologia “raccontata” all’operatore sanitario di turno; The situation may get
severely damage both the physical and psychological victim's son,
both result in the death of the latter, the author of the SMP, however, does take
victim to the lifestyle of the patient.
the choice of terminology of Munchausen syndrome by proxy
(Munchausen syndrome by proxy) is credited by scholars [5] [6] to the pediatrician Roy Meadow
St. James Hospital in Leeds, England, in 1977 public
research in which he describes two cases of SMP regarding current
creation of diseases by mothers. The first case describes the story of a mother liable
introduction of blood in the urine of the daughter of
few years, in such quantities as to affect so inexplicable values \u200b\u200bof clinical
thus inducing physicians to refer the child to numerous health treatments.
The second case, however, concerns a mother who gives his son
toxic doses of salt, thus forcing him to repeated hospital admissions and investigations
of all kinds, leading to a healing phase with relative resurgence of hospitalization
disturbances to the return to the same family;
the case is concluded, according to the story of a pediatrician, with the death of the child.
While other authors such as Ackerman, and Burman, Stevens [7] [8] abbiano
accreditata è stata Munchausen sindrome by proxy.

The DSM-IV [9] does not exempt from making the difficult diagnosis of this syndrome;
although, in fact, the disease now known to be a major focus
part of the experts, it remains little known mainly because the protagonists
of such behavior have an extraordinary capacity for simulation. The
syndrome, though suffering the initial difficulties nosographic, has received a scientific
in DSM-IV, considering
expression of a behavior disorder, inserts within the "Fictitious Disorder for
Attorney." The manual also attempts, not without effort because of the lack of
a theoretical framework within which to enter the syndrome, the identification of criteria
nosographic able to help the representation of the disease, criteria that are intentionally produced and identified in
in the simulation of signs or symptoms
physical or mental disability in a child usually small by a parent, most often the mother
. The manager, therefore, "factory" in the victim's symptoms
an actual disease or "fortify" existing symptoms. The latest edition of the Manual
da parte dei medici e dello staff ospedaliero in general, to understand the many diseases
"sham" designed specifically to give the young victims. Meadow [11]
tried in 1982 a scheme of simplification of erroneous diagnoses in head
the victim before the discovery of the definitive diagnosis of SMP (Tav.1).
The techniques used by the author of SMP are very diverse and often take on some of these traits

65 STUDIES AND RESEARCH
Here are the most common methods by which the framework can be realized
pathological falling SMP in accordance with the classification of the student
Bools:
1) short stories narrated by the author of SMP apnea: it refers to events that
ranging from seizures to the states that require cardio-pulmonary resuscitation
, these events are always described in detail by the author of medical
SMP but not supported by any evidence, even
allergie, ematuria, problemi respiratori, sanguinamenti, vomito, affezioni
gastrointestinali;
conoscenze mediche;
per il diabete, il sale comune, gli antidepressivi oppiacei medicines and poisons;
4) choking: This method consists of upper airway obstruction resulting in
apneic crisis, as stated by Merzagora Betsos [15],
about this method, you are assumed correlations between the SMP and
SIDS (Sudden Infant Death Syndrome or
Sudden Infant Death Syndrome);
5) produce symptoms by direct methods: this method is the introduction of infected material
intravenous salt, urine, feces, water, etc.
.;
6) production of symptoms through the administration of non-nutrient food and medicines
: This method includes dietary restrictions, the
no medication essential for the health of the child or the
failure of medical checks are essential to the child;
Table 3 - Most common methods of abuse in the SMP
- Smothering for airway obstruction
- Poisoning by anticonvulsants, opiates , laxatives, diuretics, barbiturates, theophylline, codeine, arsenic

- Poisoning with salt, sugar, water
- Manipulation of test samples or medical records
- Introduction of fragments of various kinds in the lower urinary tract
- Induction of fever with the introduction of immunizing substances
- Drainage of large amounts of blood
psichiatrico[16];
8) problemi fittizi in gravidanza: questo metodo comprende travagli simulati,
sanguinamenti inventati, maltrattamenti al feto;
numero eccessivo (più di cinque ma anche più di dieci sintomi diversi);
- il trattamento non ha alcuna efficacia;
con il bambino;
- il genitore ha o esibisce qualche conoscenza di medicina;
mettere in sospetto;
staff ospedaliero;
- il genitore non lascia mai il figlio da solo durante la degenza;
in famiglia.
problemi comportamentali a scuola e in casa, problemi a livello emotivo,
Infine, especially if the syndrome involves adolescents,
is not uncommon for them to develop symptoms of conversion behavior
actual collusion with their mothers in a kind of folie à deux
also developing them Munchausen syndrome. With regards the operating
of SMP in a proper icon criminal law, the phenomenological descriptions of behavior suggest munchausiani to be able to enter the case in art. 572 Criminal Code which governs the crime of abuse in the family. Even most scholars [21] [22] shares the view that the SMP can be framed from a legislative point of view in art.

S. ha una sister two years older than that, according to the report the mother, seems to be in good health. S. has a normal psychomotor development and a normal height-weight growth, does not suffer from particular diseases until in May 1992 at the age of 9 years approx. begin to experience some episodes, returning from school, in fact, the child has some diffuse erythematous spots in the chest, which necessitate the intervention of the family doctor. After a few hours after visiting the child begins to complain of headaches and nosebleeds, and then the doctor, again requested by the mother, recommended that the laboratory analysis.
During the night, the symptoms will diminish but resumed the next morning with intensification of epistaxis in addition, as reported by the mother, worsening dyspnea and neck swelling. Faced with the onset of these diseases the mother consult a pediatrician who diagnosed hypertensive crisis and prepares emergency shelter in a nearby emergency room where a CT scan is done to the skull of the mother is unable to provide the report. After the resignation of the child is subjected to several weeks to Holter monitoring unit with the pressure which does not show any significant change in blood pressure also played in the same period electroencephalograms were normal. In the following months, however, the mother reported that Silvia complains again epistaxis and headache accompanied by a state of being easily fatigued.
In January 1993, you have the emergency hospitalization at the hospital in his country for the onset of abdominal pain, and later to the suspicion of acute appendicitis is an appendectomy performed. The mother then report that to this end the doctor suggests a diagnosis of lymphadenopathy; this hypothesis, however, is only told from the mother without documentary evidence in this regard. The week following this incident S. again complaining of malaise, bloating, abdominal pain and diarrhea was admitted again to a nearby hospital for a suspected occlusion and subjected to barium enema resolution del quadro.
Nel gennaio 1995 la vittima inizia a present a symptomatology characterized by intense abdominal pain, dizziness, muscle relaxation and decreased visual acuity, since this pathological picture is repeated at a distance of about one month S. is admitted to a hospital in central Italy in the Department of Pediatrics. Haematology, radiographic and serological negative result, however, the examination
ecocuore shows a slight left-right shunt haemodynamically insignificant.
During this hospitalization the doctors perform a biopsy Jejunal
compatible with celiac disease. The girl was discharged from hospital with a diagnosis of the latter and fainting and celiac disease are advised to go to a district of Cardiology hospital in northern Italy for further testing, performed a short time, can help diagnose the nature of vasovagal syncope episodes. 69
STUDIES AND RESEARCH
In July 1995, S. is submitted at the Institute of Gastroenterology of a hospital in central Italy for a series of checks from the negative. After a few months for the persistence of symptoms, was admitted at the Department of Clinical Medicine at a hospital in Siena where they are found: the hypofunction right knee, diffuse muscle wasting, scoliosis of lower dorsal spine, generalized hypotonia but with osteo-tendon reflexes present and lively.
Following that pathological findings are made more tests: a
skin biopsy compatible with a diagnosis of Ehlers-Danlos syndrome, a capillary
that is not compatible with collagen, genetic counseling
compatible with a diagnosis of Ehlers-Danlos syndrome type IV and a dermatological consultation, the latter shows lesions related to collagen type Ehlers-Danlos syndrome. S. is therefore discharged with a diagnosis of celiac disease and collagen disease suspected to be further consultation with children.
are related hospitalizations in Padua and then in Switzerland but the mother is not asked for health records.
In the department of Pediatrics is made a new skin biopsy to exclude definitely the diagnosis of Ehlers-Danlos syndrome, after the negative health diagnostic findings agree reintegration of gluten in the diet but the mother refuses. After that the child is no longer subject to visits by children. Subsequently S. is subject to hospitalization in a Department of Rheumatology, but from now on are no longer available information on it. The mother of S. are very active and cooperative in the hospital offering the willingness to participate personally in the administration of therapy for her daughter. Socialize much with both the paramedics and medical personnel who manifests a seductive behavior especially against the physician, as he often does little gifts. It always shows careful and concerned about her daughter
but not to the point of being obsessive. An examination of the cultural background does not show
studies on the medicine despite having good medical knowledge in relation to diseases of the daughter. Towards the child seems to exert
a relationship of domination, the daughter in fact, proves to be completely dominated
mother until you get to participate actively in the illness narratives of which she is afflicted by the mother herself told the doctors.

health in the Department of Pediatrics, Hospital of Siena, in which S. was hospitalized;
according to the doctors, in fact, the clinical history reported by the mother does not fit in any way with the anamnestic history and laboratory data. The girl's mother about the behavior can be defined as "itinerant health" since the health records show that she has suffered over a relatively short time at least 12 shelters in different areas of Italian hospitals, Department of Pathology at to pediatrics, to that of Rheumatology.
The father appears to be present during the hospitalization of her daughter, is described as an organized person, good looking and seems to be younger than the wife, in fact, was very jealous.
3. REFLECTIONS ON THE CASE
The case above, we can say, did not have a conclusion in the sense of an unveiling
munchausiano mother's behavior toward the small S. but only gave rise to suspicions of pediatricians and health colleagues with whom they have consulted, which suspects the latter has not achieved any effect (reporting to the competent authorities, inquiries ,..); the mother in fact, conscious special attention of health against him by "itinerant health", he immediately removed from his hospital address to Siena. But the case has left a considerable confusion in the nursing staff who for various reasons was compared with the mother, some of them in fact, have expressed feelings of disappointment with regard to the mother of S. that seemed very "affectionate" with his daughter, grateful to the nurses with whom he established friendly relations even during the period of hospitalization
daughter. The feeling of disappointment, combined with the anger of having been manipulated by the mother is a known in the literature, some American researchers have clearly shown this [23].
The mother is the protagonist of this case have all the features found in the literature and observation di casi clinici di SMP, ovvero una donna collaborante con gli operatori sanitari, piacevole, premurosa e piena di attenzioni nei confronti del figlio, perennemente accanto al figlio nel corso della sua ospedalizzazione[24], grata nei confronti dei medici che curano il figlio, comportamento questo, che li spinge sempre ad approfondire le procedure sanitarie.
Quanto alla figura del padre, il caso evidenzia certamente differenti modalità comportamentali rispetto ai casi osservati in letteratura; generalmente infatti, le figure paterne nei casi di SMP sono quasi sempre assenti dalla vita familiare, deboli, negligenti, con ruoli del tutto passivi ed insignificanti[25], tale assenza va interpretata anche nel senso del mancato coinvolgimento da parte of his wife so he can act freely on the child without interference. In the literature [26] are found in very few cases where the father engaging in conduct assignable to the syndrome which is spoken;

4. CONCLUSIONS
The review of cases reported in the literature on the subject of much research and it shows in reference to the phenomenon of which we are dealing, two dramatic situations: on one side of your ignorance on the subject of those health workers who SMP should not only know the existence nosographic also be provided with adequate technical knowledge on how the implementation of the same syndrome, other the difficulty of diagnosing the SMP as the clinical picture in which the disease presents is extremely diverse, as diverse and multifaceted is the fantasy of the mother that creates the situation munchausiana.
Another difficulty is the psychiatric classification in terms of whether or not the mother's behavior since that certainly belongs to the learn clinical syndrome, in this regard, it should be stressed that most scholars [27] believes that mothers do not present the protagonists of the SMP psychiatric pathologies of considerable gravity but rather are found personality disorders (Histrionic, Borderline, Paranoid).
madre/figlio - qui svolta in termini distruttivi - e dall’altro however, because the behavior
munchausiano integrates and relies on the rigor of the criminal law. These issues
suggest a way forward that is working to support the
training needs of those persons for their own profession are the possible referents
of that 'help' unexpressed or not grasped the
mothers with this syndrome. E 'on the ground then the loneliness of his mother affected by the SMP must act
equipping the possible referents of the' help 'to see the signs of disease
in place because of the great ally is the mother munchausiana
medical doctor or agent distracted or even worse ignorante del fenomeno e che
