Today, February 23, 2011, in the online journal PLoS ONE, researchers from six institutions have published a fascinating study that uses the powerful capabilities of investigative technology to catalog the proteins in samples of spinal fluid in Chronic Fatigue Syndrome (CFS) and Lyme disease, neurological post-treatment. The exciting work is the product of a great team headed by Steven Schuzter University of Medicine and Orthodontics in New Jersey and Thomas Angel and Tao Liu della Pacific Northwest National Laboratory.

This combination of methods was selected for its ability to launch a broad network of "discovery" in the analysis of complex biological samples, without having to define a priori which proteins might be present. The group had previously identified a "proteome" including normal as a reference point for comparison with samples infected. The term proteome refers to the entire range of proteins found in a biological sample, in this case, the cerebrospinal fluid. Proteins are made from smaller particles called peptides.
Using this analysis, the research team was in grado di generare una lista comprensiva di 30.000 peptidi nei campioni raccolti dai partecipanti in ciascun gruppo di malati. Esaminando le vie biologiche implicate nelle varie proteine identificate, il gruppo di ricerca ha scoperto che le proteine nella cascata complementare erano abbondantemente elevate nei campioni dei pazienti con la Malattia di Lyme post trattamento e la CFS rispetto al gruppo di controllo, ma a livelli diversi tra loro. Il sistema complementare fa parte del sistema immunitario e serve a sgomberare patogeni infettivi. Curiosamente, nel 2005, una ricerca sui proteomi condotta da James N. proteins involved in Cdk5 signaling pathway were significantly richer in patients with CFS. Alterations in Cdk5 activation pathways have been linked to Parkinson's disease and Alzheimer's. The proteins involved in specific neurological functions were lower in patients with CFS compared to patients with Lyme disease after treatment, and both groups had lower levels than the control group. The authors acknowledge that the clinical significance of the identified proteins and their levels in the control group is "difficult to determine the stage of discovery."
Votes also examined individual samples of FCS (instead of pooled samples) of participants with CFS and Lyme disease after treatment. In doing so, they identified a range of proteins that distinguish the two states of disease (Figures 2A and 2B). By analyzing individual samples of FCS, they determined that patients with Lyme disease can be distinguished from patients with CFS. The ability to distinguish between CFS and Lyme disease after treatment on the basis of these proteins has important diagnostic implications. Since the causative agent for Lyme disease is known as Borrelia burgdorferi, this finding suggests that there are distinct pathophysiology for these two clinically similar diseases.
